Why India for Liver Transplant??

Why India for Liver Transplant??

Liver transplantation is currently in its golden period in India. The number of transplants being performed and the steady increase in the new programs that have emerged over the last decade is a testimony to it. To date, the majority of transplants performed in India are live donor liver transplants. Deceased donation is more sporadic and concentrated in a couple of regions. With phenomenal increase in transplant activity in India, there is huge potential for streamlining data sharing among programs in India and with the rest of the world to ultimately benefit the transplant community.

Innovations are an integral part of growth and LDLT in India has followed the norm. Organ shortage is a universal problem in transplantation and ways to tackle it has always been an area for innovation. The large number of transplants in India allow for the possibility of paired exchange either for size mismatch or for blood group incompatibility.

Another advantage is the technical advancements like technique of arterial anastomosis in auxiliary LDLT, use of recanalised umbilical vein graft for anterior segment reconstruction, middle vein clamp technique to determine need for anterior segment reconstructions have been widely used and documented from programs in India. These innovations establish the maturity and technical expertise achieved in LDLT in India. Pediatric transplant definitely calls for more surgical and intensive care expertise and the number of pediatric transplants done in India in 2017 was around 65 with our program contributing to 42 pediatric transplants. At this rate, India is soon expected to be one of the largest pediatric transplant centers in the world.

Liver transplantation as an established form of treatment for end‐stage liver disease has gained acceptance in India over the last 10 years. Liver transplantation in India has unique features that have contributed to the growth of both deceased donor and living donor transplantations.

India has now become a major center for LT for international patients because of the high quality-low cost value proposition. Children from over 20 countries have now received a LT in India (personal communication). Withincreasing number of children crossing the 5 and 7-year post LT landmarks, 10- year survival data from India is now awaited and will become available in the next couple of years.

  1. When should a liver transplant be performed?
  2. When a person’s liver is severely damaged and cannot function properly or complications may develop and liver transplantation should be considered. Conditions like hepatic coma, massive upper gastrointestinal bleeding, and liver cancer is best treated by complete removal of the liver (cirrhotic liver). In general, when a patient needs a new liver, the earlier the operation, the higher the success rate is.

  3. Who can be a suitable living donor?
  4. The most important criteria isthat the donation of portion of the liver is done voluntarily. The donor has to be less than 50 years of age, body mass index of less than 25 and is a near relative of the recipient. Both the donor and the patient should have the same blood group. Besides, the potential donor should understand clearly that

    • The donor operation carries complication rate of 10-15%.
    • The recipient is successful in 90-95%, which means that there is 5-10% chance of dying.
    • The donation is done out of his/her own wish and without any coercion.
    • There is no financial gain related to the act of donation.
    • The donor has the right to withdraw at any time without the need of giving any reasons to do so.
  5. Which patients are excluded from liver transplantation procedure?
  6. Patients who have cancer in another part of the body, active alcohol or illegal drug abuse, active or severe infection in any part of the body, serious heart, lung or neurological conditions or those who are unable to follow doctors’ instructions are excluded generally.

  7. Can a person donate a portion of their liver?
  8. This is the most common form of living donation. Individuals can donate a portion of their liver which is then implanted into the recipient. The liver cells regenerate after the donation until it has regrown to almost its original size in both the donor and recipient.

  9. How painful is Liver Transplant?
  10. There is pain after liver transplant surgery, however it is generally not as severe as with other abdominal surgeries. This is because nerves are severed during the initial abdominal incision causing numbness of the skin around the abdomen. These nerves regenerate over the following six months and sensation returns.

  11. What happens after a liver transplant?
  12. Your doctor will take out your diseased liver when you get a donor liver. Your belly and side will be sore for the first 1 to 2 weeks after surgery. You also may have some numbness around the cut (incision) the doctor made. It is common to feel tired while you are healing.

  13. Does the donor need to be a relative?
  14. Living liver donors and their recipient do not have to be related. Any healthy person can give the gift of life

  15. What are the costs for the donor?
  16. The recipient's insurance pays for all live liver donor medical expenses related to donation. Travel costs and time away from work are often not covered by insurance and need to be considered. Costs to the donor include:

    • Time off work
    • Medications such as pain pills after the surgery
    • Transportation for visits
    • Non-hospital lodging costs, if needed
  17. What are the risks involved with being a living liver donor?
  18. It is common for people to ask if it is safe to donate part of their liver. While the results of adult-to-adult living donor liver transplantation throughout the world are very good, the procedure is not without risk to the donor. Potential risks of this surgery include, but are not limited to, the list below.

    • Blood clots in the legs or lung
    • Fluid retention (edema)
    • Bile leakage or bile duct complications
    • Hernia
  19. What Happens when a person is on Transplant Waiting List?
  20. ntly there are more people who need a new liver than there are donors. Therefore, most people will be placed on a transplant waiting list. While if someone is on the waiting list, the transplant team will regularly monitor their liver’s health and provide other treatments if needed, such as surgery, embolization, or ablation.

  21. What Happens During Liver Transplant Surgery?
  22. During transplant surgery, the transplant team replaces the diseased liver with a donated, healthy liver. The blood type and body size of the donor have to match the person receiving the transplant. Liver transplant surgery usually takes between 4 and 12 hours. Most patients stay in the hospital for up to 3 weeks after surgery.

  23. What Happens After Transplant Surgery?
  24. After the transplant surgery, a patient may need to stay in the hospital for several weeks while their health care team monitors how well body is accepting the new liver. Patient will take medicines to control any pain or will start taking immunosuppressive drugs to prevent body's immune system from rejecting the new liver.

Patient will need the most medicines during the first 3 months after transplantation. After a liver transplant, patient will undergo regular blood tests and occasional liver biopsies to make sure body is not rejecting the new liver. In addition, these tests will show whether the liver cancer is in remission or returning.

  25. Is there an age limit for liver transplantation?
  26. The age limit is individualized as it varies with a patient's overall health condition. However, it is rare to offer liver transplant to someone greater than 70 years old.

  27. What factors are considered when choosing a liver transplant center?
  28. The most important factors in choosing a liver transplant center for a person are the team’s experience and expertise. You should be looking for a program that performs a high volume of transplants each year, with experience in complex pediatric liver transplant surgeries.

  29. How long is the liver transplant surgery take place?
  30. The liver transplant procedure generally takes between four and tweleve hours. The patient will come to the hospital, often the night before, then goes down to the operating room, often for about two hours of anesthesia before the surgery even begins.

  31. What are the stages of liver disease?
  32. There are different ways in which the liver disease stages occur depending upon the specific liver disease involved. Basically, there are four different condition that affects the liver’s health and affect the liver’s functions:

Stage 1 : Inflammation of the liver Fibrosis of the liver : In this initial stage, pain in the stomach can be witnessed as the body tries to fight the infection. If not treated at the earliest, it can worsen the liver condition. In some cases of liver disease, the symptoms and inflammation treatable for preventing from progressing to the second stage.

The most common reasons can be alcohol and chemical abuses.

Stage 2 : Fibrosis of the liver : In this stage, the damage or scarring from the first stage begins to hamper the blood flow of the liver. This usually happens when the liver is inflamed for a long period of time. In this stage, the liver does not function properly but through treatment it might be able to heal not allowing any further progression in the disease.

Stage 3 : Cirrhosis of the liver : In this stage it creates permanent scarring that block the blood flow. This is a dangerous stage that causes other serious conditions and symptoms that leads to worsening of the disease. At this stage, doctors focus their treatment on managing the symptoms.

Stage 4 : Liver failure and advanced liver diseases : This is the final stage of the disease, in which the liver fails to perform the normal functions and now the patient needs urgent medical treatment to prevent death. Symptoms of liver failure can be vomiting, fatigue and diarrhea as well as symptoms from stage 3.

The progression from cirrhosis to complete liver failure might take years and the damage caused is irreversible ad eventually leads to death.
