My father is 68 years old and was diagnosed with Papillary (RCC)Renal Cell Carcinoma Type 1. Our doctor said this is a stage 4 kidney cancer as they've found a similar mass in the lungs. Now the doctor has prescribed us to go with Targeted Therapy with a tablet called Sutent. The tablet price is too expensive for us and we cannot afford it. With a little research, we've found there is a tablet called Votrient which is quite similar to this and is affordable. We just would like to get an opinion from a Medical Oncologist who can help us understand which medicine is suited for my father's condition. Appreciate your help in this regard.

Dear Chhaya, If cancer has spread and cannot be removed, the following types of palliative surgery may relieve symptoms:
- Biliary bypass: If the tumor is blocking the bile duct and bile is building up in the gallbladder, a biliary bypass may be done. During this operation, the doctor will cut the gallbladder or bile duct in the area before the blockage and sew it to the small intestine to create a new pathway around the blocked area.
- Endoscopic stent placement: If the tumor is blocking the bile duct, surgery may be done to put in a stent (a thin tube) to drain bile that has built up in the area. The doctor may place the stent through a catheter which is a soft hollow tube which is passed into the bladder that drains the bile into a bag on the outside of the body or the stent may go around the blocked area and drain the bile into the small intestine.
- Percutaneous transhepatic biliary drainage: A procedure done to drain bile when there is a blockage and endoscopic stent placement is not possible. An x-ray of the liver and bile ducts is done to locate the blockage. Images made by ultrasound are used to guide placement of a stent, which is left in the liver to drain bile into the small intestine or a collection bag outside the body. This procedure may be done to relieve jaundice before surgery.

I have a left testis seminoma and that will be out after the orchiectomy operation. Now the doctor told me to take chemotherapy with a low dose, can it be treated by targeted cancer therapy, and is it required to store the sperm for fertility as one testis is present?

Hello Abhijit, Chemotherapy has to be taken and there is no role of Targeted or Immunotherapy in testicular cancer. For the treatment, the age of the patient also has to be considered. You will have to store your sperms before treatment begins. We request you to kindly send us your test reports by clicking on the Whatsapp icon on the right side of the page so that we can guide you regarding the line of treatment in the best way possible. My mother-in-law whose age is 49 years is suffering from jaundice for the last 1 month (bilirubin level is 20.8). The doctor has diagnosed it as a malignant neoplasm of the Gallbladder (according to other doctors hepatobiliary CA). Is this curable with surgery or medication? Please Suggest.

Answered on 23rd May '24
Hello Avijit, Hepatobiliary CA needs a multidisciplinary approach. The prognosis in such cases is not good. Treatment plan mainly depends on the extent (stage) of the disease, Investigations include Blood tests, Imaging tests, liver biopsy. After all the parameters being considered a medical treatment like Radiation therapy, Targeted drug therapy, Immunotherapy, Chemotherapy and if needed surgery will be decided. We advise you to consult an oncologist as only after clinically examining the patient, the doctor can guide you further. Wish your mother-in-law a speedy recovery. Hope our answer helps you. Hello, I have been diagnosed with Squamous cell carcinoma in mouth. Please suggest the best possible treatment.
General Physician
Answered on 23rd May '24
Squamous cells is a thin, flat cells lining inside of the lips and oral cavity. The Cancer that grows in this cell is called squamous cell carcinomas. Squamous cell carcinoma usually develops in areas of leukoplakia (white patches of cells that do not rub off). Treatment options for squamous cell carcinoma depends on the stage of cancer, size of tumor and location of the cancer (where it is in the lip or oral cavity), also whether the patient's appearance and ability to talk and eat can stay the same, as well as their age and general health. Patients with lip and oral cavity cancer should have their treatment planned by a team of doctors who are expert in treating head and neck cancer. Two types of standard treatment are used: Surgery, Radiation therapy. Consult Oncologists in Mumbai, or in any other city.

41 year old lady underwent laparoscopic cholecystectomy for gallbladder stones, post surgery if biopsy turned to be cancer, we need to evaluate and treat further. My question is what else treatment u had after diagnosis of cancer. Usually we do PET CT scan to know stage followed by radical cholecystectomy for gallbladder cancer. Frankly speaking gallbladder cancer per se has poor prognosis only

I have high grade squamous intraepithelial lesion. Which hospital or Doctor is recommended for such treatment?
Pankaj Kamble
Member of team ClinicSpots.
Answered on 23rd May '24
The presence of High Grade Squamous Intraepithelial Lesion (HGSIL) means the presence of pre-cancerous cells (NOT cancerous cells). As a course of treatment, the doctor would remove the abnormal cervical tissue to avoid cervical cancer.
The treatments used could be:
- Loop Electrosurgical Excision Procedure (LEEP)
- Cryotherapy
- Conization
- Laser Therapy
For getting treated you should consult a Gynae Oncologist. If the finances are tight and you do not mind a long waiting time for treatment, it would be best that you visit charitable/ government hospitals which would give you affordable treatment.
In Delhi, they are:
- Rajiv Gandhi Cancer Institute and Research Centre, Rohini (Charitable)
- AIIMS Hospital, Ansari Nagar (Government)
If you do not want to wait for a long time and are ready to pay a premium for your treatment, then you can go to a Private Hospital. In your case preferably a hospital with a dedicated Gynae Oncology department. Some of the best private hospitals for your treatment in Delhi are mentioned in the following page: Cancer Hospitals in Delhi. Hope this has answered your question completely.

Hello Sachin, Treatment for oral cancer (mouth cancer) or any other cancer normally depends on the type, location, and stage of the cancer, age of patient and patient's general condition when diagnosed.
Oral cancer treatment includes:
- surgery in early stages,
- Radiation therapy,
- Chemotherapy.
- Treatment for advanced stages will usually involve a combination of chemotherapy and radiation therapy.
- Targeted therapy can be effective in both early and advanced stages of cancer.
In your case, depending of stage of cancer or if it is recurring, the clinician will decide about the line of treatment. Very important is nutrition of patient as eating will be a concern during and after treatment for some time. Hygiene of mouth is also very important and not to be missed. In case of advanced oral cancer, the patient may need reconstructive surgery and some rehabilitation to assist with eating and speaking during recovery. Speech therapist, nutritionist will be needed. Kindly consult an oncologist for evaluation.

Hi, Patient's age is 73 years diagnosed with Cancer prostate and bone metastasis. What is the next treatment? Already underwent angioplasty before 4 years.

Answered on 23rd May '24
Hello Moncy! In this case, as you say bone metastasis has occurred then the prostate cancer is above Stage 2. In higher stages of prostate cancer or metastatic prostate cancer by treatment you can only slow down the spread of the cancer but there is no cure to it.
Prostate cancer treatment options which can slow down the spread are as follows:
- Hormone Therapy: This treatment is also known as Androgen Ablation Therapy in which they suppresses testosterone formation which reduces the spread and formation of the cancer causing Prostate Specific Antigen (PSA)
- Chemotherapy: If the patient does not respond to Hormone Therapy then chemotherapy is administered. Here they will give the patients anti cancer drugs like Docetaxel via Intra Venus line (IV).
- Immunotherapy: They will inject an injection like Provenge which would have antibodies into the patient to prolong their survival.
Hope my answer has satisfied your question! You can find hospitals for this disease on our page - Prostate Cancer Treatment Hospitals in India.
I have mouth ulcers in my mouth and tongue, pain in the neck and right ear. Is it cancer? Which doctor, should I consult first?
Pankaj Kamble
Member of team ClinicSpots.
Answered on 23rd May '24
It is not necessary that all mouth ulcers lead to cancer. They have multiple causes like a viral infection or stress. I am sure that it is not cancer. But to be on the safe side, it is best that you consult a doctor. To start with, you should contact your family physician who will guide you about your condition. Further, he may refer you to an Oncologist, if he feels you have cancer. Mouth cancers are usually treated by a Surgical Oncologist or a Maxillofacial surgeon having experience in head and neck cancer treatment. If you're looking for which doctor to consult for tongue can also find physicians on our page - General Physicians in India.
