"As" per your clinical history is concerned please take these medications,

1) Metronidazole 400mg twice a day for 7 days,

2) Fluconazole 150mg once a day for 7 days,

3) Clotrimazole 100mg vaginal tab once a day at bedtime for 6 days, 

wash vagina in diluted apple cider vinegar solution twice a day for 30 days.
