Why am I experiencing dizziness, breathlessness, weakness, tiredness?

I often feel dizzy ,experience shortness of breath, weak and tired after a short walk.I also experience a loss of apetite and a loss insight.Please diagnose and tell me what to do.,provide a prescription if possible Thanks in advance
