I am suffering from Lumbar Radiculopathy. For the past 1 year, I am suffering from back pain and I have taken medicines, physiotherapy but can't get relief in pain. Please help.

Hello, Lumbar Radicular symptoms include pain which radiates into the lower extremity (thigh, calf, and occasionally the foot) directly along the course of a specific spinal nerve root. The most common symptom of radicular pain is sciatica (pain that radiates along the sciatic nerve - down the back of the thigh and calf into the foot).

Treatment:

  1. In the initial stage, the doctor suggests a nonsurgical treatment (such as physical therapy, medications) which often gets improve within 6 weeks to 3 months.
  2. Since in your case the nonsurgical treatment does not alleviate the pain, decompressive surgeries such as laminectomy and/or discectomy/microdiscectomy. These surgeries typically provides relief of radicular pain for 85% to 90% of patients.

We suggest you to consult a nearby Orthopedic and only after a physical examination, the complete treatment can be provided by the doctors. I hope this answers your query. Find our page which will help you look for doctors - Orthopedic Doctor in India.

I had an accident in 2005 August, I had a brachial plexus injury, I can't move my left hand. My left shoulder, wrist, elbow fused in CMC Vellore 10 Years ago. Any further treatment in India?

Hello Faisal, treatment depends on several factors including the severity of the injury, the type of injury, the length of time since the injury, and other existing conditions. And since you have mentioned it's been 10 years, so the reason is nerve tissue grows slowly, so it can take several years to know the full benefit of surgery.

Further, there are various surgeries we have mentioned below:

Nerve graft: In this procedure, the damaged part of the brachial plexus is removed and replaced with sections of nerves harvested from other parts of your body. This helps restore your arm's function.

Nerve transfer: When the nerve root has been torn from the spinal cord, surgeons often take a less important nerve that's still working and connect it to a nerve that's more important but not working.

Muscle transfer: Muscle transfer is a procedure in which your surgeon removes a less important muscle or tendon from another part of your body, typically the thigh, transfers it to your arm, and reconnects the nerves and blood vessels supplying the muscle.

Hi, I need urgent treatment for vestibular migraine. Please guide me with its treatment.

Hello,

  • There's no specific medication for vestibular migraines. Your doctor will prescribe different drugs to stop an attack when it happens, same is called abortive therapy:
  1. Triptans: this medicine is taken at the first sign of headache symptoms.
  2. Vestibular suppressant: It can ease your dizziness and motion sensitivity. This type of drug works on the balance center in your inner ear. Your doctor might prescribe benzodiazepines like lorazepam (Ativan), anti-nausea drugs like promethazine (Phenergan),
  3. Antihistamines: like meclizine.

  • If you have frequent or disabling vestibular migraines, your doctor may try drugs similar to traditional migraine prevention meds. These include:
  1. seizure medicines,
  2. blood pressure medicines (like beta blockers and calcium channel blockers),
  3. and some antidepressants.

  • CGRP inhibitors: are a new class of preventive medicine that your doctor may recommend if other medicines don't help.

We suggest you consult a doctor regarding migraine treatment, before going for any medicines, as they prescribe you the medicine according to your health condition, and our page might help with that - General Physicians in India. Hope this answers your query.

I am suffering from thyroid, still can I go for IVF treatment?

Hello Sruthi, We would recommend that before going under any infertility treatment like IVF, ICSI, etc. thyroid hormone levels (TSH) must be brought into balance because it is important to keep your TSH (thyroid stimulating hormone) at optimum level as it affects ovulation. Further, we would suggest you consult this issue in some of the reputed clinics of Mumbai which is mentioned below: Ivf Centers in Mumbai. Hope this answer helps you.

My age is 45 years and I got married late. This is my first marriage and I want to go through IVF treatment.

Hello Ritu, To undergo IVF treatment after 40 will generally, be dependent on your own specific circumstances. By mentioning specific circumstances, I am considering both your fertility and medical history. The IVF doctor might ask you to get a few tests done to understand the exact reason for not conceiving naturally. If he recommends IVF then the process will be as follows:

  • IVF Treatment: In a standard cycle of IVF, you will have to take medication for 7-10 days to stimulate your ovaries to grow and also mature multiple egg follicles. Under anesthesia, the eggs will be retrieved and then they will be combined with the sperms of your partner. They will be left overnight for fertilization to take place. If the eggs are fertilized then the resultant embryo will be transferred to your uterus after 3 to 5 days. And after around two weeks you will have to undergo a pregnancy test to verify whether you have conceived or not.
You can read our blog if you wish to understand IVF success rate, IVF process step by step, and requirements for IVF - In Vitro Fertilization.

  • For further knowledge regarding this treatment, you can consult the renowned Doctors of Mumbai listed on our page - Reproductive Endocrinologist (Infertility)s in Mumbai.

    Hope this answer helps you.

Hi, I am a male with azzospermia problem and I have a failed ivf procedure this year and want to try another option in India. Which would you recommend ivf or icsi?

Hello , As you are already suffering from azoospermia problem ICSI would likely be a better option compared to IVF. Also, your past history with IVF treatment didn't work so it is quite clear that ICSI may give successful result in the future. You can also consult the doctor about IMSI treatment (Intracytoplasmic morphologically selected sperm injection) which can be also considered in the case of Azoospermia. In this technique sperm cells are examined using an advanced and a highly magnified microscope that helps in the selection of better sperm that is having the highest chance of achieving successful fertilization. Hope this answer helps you. Our blog will help answer your questions with regards to other assisted reproductive techniques - Cost of IVF Treatment in India.

Hi Sir, I have been operated for an ectopic pregnancy 4 years ago. I'm now 35 years old. My hormonal profile and my husband's sperm analysis is normal. HSG showed a fimbria end block. What options should I consider for fertility?

Have you checked the AMH level of yourself, and also the antral follicle count in Sonography?

Hsg is not the absolute report, probability of it being right is 60% since the patient is conscious and the procedure itself is painful, thus the report can falsely show positive/negative indication. The true status of the tube is better assessed by a diagnostic hysterolaparoscopy, wherein we put a telescope in your abdomen.

If in any doubt, then approach doctors from this page - Ivf doctors in India, or you can seek help from me too.
