Walk into any commercial gym on a Monday morning. There's a reasonable chance one in five people you see is using, or recently used, some form of peptide therapy. Semaglutide for weight loss. BPC-157 for a nagging shoulder. Ipamorelin for recovery. NAD+ because a podcast promised it would reverse aging.
The peptide market isn't emerging. It's mainstream. According to Grand View Research, the global peptide therapeutics market was valued at $140.9 billion in 2025 and is projected to reach $294.6 billion by 2033. And at the consumer level, it's almost entirely unverified.

Multiply that by every metro in the country.

There is no public registry, certification, or independent verification that lets a consumer check whether a peptide provider has real medical oversight, uses quality-controlled pharmacy sources, follows a legitimate prescribing process, or even employs a licensed prescriber.
So Paepke built one.
What the Screening Found

1. Who is your medical director, and what is their license number?
No immediate answer means no appointment.
2. Is this FDA-approved or compounded?
Neither answer automatically disqualifies a provider. Not knowing does.
3. Which pharmacy compounds it, and is it licensed and FDA-registered?
Refusing to name the pharmacy is the single biggest red flag in the entire process.
4. What is the prescribing process?
Legitimate care means a real consultation, full health history, and usually labs—before anything ships.
5. What are the risks and side effects?
If the conversation only includes benefits, the patient is being sold to, not treated.
The Window Is Already Closing
This is not hypothetical anymore. The FDA declared the tirzepatide shortage resolved in December 2024 and the semaglutide shortage resolved in February 2025, ending the exemption that had allowed compounders to mass-produce copies during the shortage period.
The regulatory tightening didn't stop there. In April 2026, the FDA proposed removing semaglutide, tirzepatide, and liraglutide from the 503B bulks list entirely, finding no current clinical need for outsourcing facilities to compound these medications in bulk. The agency has also issued dozens of warning letters to compounding pharmacies and telehealth platforms making false or misleading claims about compounded GLP-1 products.
The safety concerns behind that regulatory pressure are real. One published study found that compounded GLP-1 receptor agonists are associated with a greater risk of adverse events, safety concerns, and product quality issues compared to non-compounded products a finding that prompted several major medical societies to recommend against their use altogether.
Patient-specific 503A compounding remains legal in narrow circumstances. A lot of what is being sold in strip malls and Instagram DMs does not meet those circumstances.
The Good Actors Deserve to Be Found
Here is the part that tends to get lost in the broader alarm: there are peptide providers doing this correctly. They have physician oversight, transparent sourcing, rigorous intake processes, and informed patients. The problem is that the current market gives them no way to prove it and gives patients no way to find them.
