Hello Doctor, good evening. I am Lokesh, and I am taking care of my grandmother at home. She has stage 4 cervical cancer, and we are currently focusing on palliative care. Unfortunately, we do not have regular access to a nearby doctor or hospital, so I’m managing everything myself at home. I’m doing my best to support her with love, comfort, and the correct medicines. She has been experiencing: Pain (sometimes mild, sometimes strong) Lack of sleep or restlessness Occasional constipation and vomiting feeling I have some medicines like: Tramadol 50 mg Paracetamol 650 mg Lorazepam 1 mg Pan 40 Ondansetron 4 mg Lactulose syrup Based on her condition, I would like to know: 1. When and how to use Tramadol or Paracetamol for pain. Can they be used together? 2. Is Lorazepam safe to give daily? Should I reduce it slowly if I stop? 3. What are the risks if I stop Tramadol or Lorazepam suddenly? 4. Can I use Ondansetron only when vomiting starts? Or daily? 5. How often should I give Lactulose syrup if there’s no bowel movement? 6. Do you recommend any vitamins or supportive care for comfort or strength? 7. If she is calm and has no symptoms, can I skip some medicines? I want to provide her full comfort, dignity, and peace. Kindly guide me with the best possible medicine routine and precautions I should follow at home.

You can give Tramadol and Paracetamol together if the pain is intense, but it's good to space them out a bit. Paracetamol can be used every 4-6 hours, and Tramadol can be taken every 4-6 hours as needed. Lorazepam can be used daily for anxiety or restlessness. If you ever need to stop it, it's best to reduce the dose gradually to prevent any withdrawal symptoms. Suddenly stopping Tramadol or Lorazepam can cause withdrawal symptoms like increased pain, anxiety, or even seizures. So, it's safer to taper them off slowly. Ondansetron can be used as needed when vomiting starts. It's usually taken 30 minutes before meals that could trigger nausea. Lactulose syrup can be given every 8-12 hours if there's no bowel movement. Make sure she's drinking enough fluids to help it work better. A daily multivitamin can be beneficial for overall health and strength. It's good for supporting the body during this time. If your grandmother is comfortable and symptom-free, you can skip some medicines. But always consult with an oncologist before making any changes.

Treatment options also depends on several factors like the tumor subtype, hormone receptor status, stage of the tumor, patient's age, general health, menopausal status, and preferences. The presence of known mutations in inherited breast cancer genes, such as BRCA1 or BRCA2. There are some general steps commonly preferred for treating early-stage and locally advanced breast cancer. Doctors generally recommend surgery to remove the tumor in breast. Though the goal of surgery is to remove all the cancer which is visible but microscopic cells sometimes remain behind. Therefore another surgery may be required. For cancers which are large or are rapidly growing, the clinician suggest systemic treatment with chemotherapy or hormonal therapy before surgery. This is called neo-adjuvant therapy. This helps in shrinking the tumor which is easy to operate; also breast can be preserved in some cases. After surgery it is important to check for recurrence. Then adjuvant therapy is advised. Adjuvant therapies may include radiation therapy, chemotherapy, targeted therapy, and/or hormonal therapy When surgery to remove the cancer is not possible, it is called inoperable, and then Chemotherapy, targeted therapy, radiation therapy, and/or hormonal therapy may be given to shrink the cancer. For recurrent cancer, treatment options depend on how the cancer was first treated and the characteristics of the cancer. Hi, I have a doubt, is there any specific reason for which immunotherapy is not recommended in the early stages of any cancer?
Chemotherapy works by stopping or blocking cancer cells from growing and immunotherapy works by boosting up a patient's immune system to find and then attack cancer. Though immunotherapy is gaining popularity but still it is in growing stage.
Chemotherapies have been key to cancer treatment from a very long time with all established and compiled data regarding the effects and side effects thus making doctors more confident about it in comparison to immunotherapy which is still newer. Ive always had discharge for as long as I can remember and at my 8 week postpartum checkup the doctor checked me but said it wasn’t alarming because it’s not bothering me. I am currently 4 months postpartum and noticed I was getting discharge that had a slight odour and the discharge left rashes between my thighs and it got to the point I couldn’t wear underwear because the discharge would get more and I keep getting rashes. I noticed it getting a little bit better when I stopped wearing underwear the smell was still slightly fishy but not too terrible like before but recently after sexual intercourse I bled a little. Now google says it’s either the C word or some infection. I’m aware I should go doctor right away but I’m just not able to do so , my last two screenings for cervical cancer with my Pap smear came negative it was in 2018 and 2021. What’s the reason I bled?
