Cystic acne at the age 0f 18 years indicates underlying hormonal cause like PCOS, insulin resistance etc. this can be evaluated by certain blood tests and scans. Please consult an experienced dermatologist for the same. Intra lesional triamcinolone injections, oral retinoids, oral contraceptive pills,  etc may be recommended once the cause is established by the Dermatologist.
