GREETINGS, I have previously taken medicines for my forgetfulness as I can't remember and forgetting simple things. All those medicines made my conditions worse.I also have periodic migraine (once in a week) . But I am actually worried about my brain . Always getting confused in words like weak and week, not able to recall words fast when I need it ( example : I remembered a word after 3 days but when I wanted I didn't get). Previous president’s name I remembered after 7,8 hours without anybodys help. Forgets names , days , dates. Diagnosis on Admission Recurrent Hypoglycemia/ Hypoxic Brain Injury/ AKI HAP History (non-compliant to medications), history of multiple falls ros Her in right shoulder fracture. S/P right shoulder ORIF on 8/1/25 admitted with the complaint of: ALOC, GTCs 2-3 epis 45 years old female patient, known case of hypertension ( shortness of breath and vomiting, On arrival in the ER her GCS was 6/15 and she was drowsy. Sho was intubated i ER due to low GCS and severe respiratory distress, H slightly deranged LFTS. Patient was started on broad s initial lab workup showed increased inflammatory ma onboard. Patient was suspected as PE for which CTPA was done which was normal. Further MRI brain screening spectrum antibiotics. Neurology and Pulmonology was t done which showed hypoxic brain injury. Her tracheostomy was done on 17/1/25. Currently GCS 5/10. Patie currently vitally stable. Ongoing IV Antibiotics. Now she is planned for discharge on home nursing care. Investigation Result Can be obtained from record room on request.

Hello, my Mother in law (70 years) is suffering from misbalancing and in-coordination of feet movement which has severely worsened during last 3 years. All the pathology test appears are normal. The sensory test is also normal. There is an uncontrolled trembling that occurs frequently. Now, this symptom is gradually being observed in the upper limbs also. I am 42 year old, having bouts of severe headache prominently on right eyebrow and temple, severe right neck and shoulder spasm, was on gabamax nt 50 for 6 month which was advised by an orthopedic doctor.
