Why does my ejaculation feel painful and include blood?

For 2 years I've been having symptoms of painful ejaculations - it feels like my urethra closes for a few seconds while I ejaculate, I start feeling pain and after that semen comes out. Sometimes, after ejaculation there's a bit of blood. I've done tests for sperm and urine and they're clean, no UTIs, infections or STDs, and my prostate is not enlarged. I've been to multiple doctors and they all prescribe antibiotics which don't help at all, ever - no change of symptoms from antibiotics. The only pill that ever helped (briefly) was betamsal (tamsulosin). After ejaculation, peeing sometimes is painful and slow.

You are having some problems with ejaculation, sometimes experiencing pain and even blood afterward. Even though your tests are negative, such symptoms might be caused by other conditions such as urethral stricture or pelvic floor dysfunction. These conditions can sometimes be unnoticed in routine checkups. Go to a urologist for further assessment and possibly get some treatments that are specifically designed for your condition.
