  • Stage 3 bladder cancer is of two types:
  1. Stage 3A: the cancer cells have reached outside the bladder to uterus or vagina but has not spread to nearby lymph nodes and tissues or organs.
  2. Stage 3B: the cancer cells have reached outside the bladder to uterus or vagina and also, has spread to nearby lymph nodes and tissues or organs. Though stage 3 bladder cancer is advanced, it can be successfully treated with surgery in combination with other therapies.
     
  • Bladder cancer treatment options are as followed:
  1. Radical Cystectomy: It involves the removal of the bladder and nearby lymph nodes. Also, uterus, ovaries, fallopian tubes, urethra, and vagina are often removed.
  2. Chemotherapy: It is given before surgery, for the purpose of shrinking tumors and post surgery to destroy any cancer cells that are remaining.
  3. Radiation Therapy: This treatment is used to kill cancer cells within the bladder and the places where it metastasized.
  4. Immunotherapy: This treatment is used to boost the immune system, so as to fight cancer by placing the drug directly into the bladder, through a catheter.
  5. Targeted therapy: In this treatment, a drug is given orally that targets cancer's specific genes, proteins or the tissue that contribute to cancer growth and survival.

Sir, I am from Kolkata. My father is suffering from Adenocarcinoma (Biliary type). Surgery (Cholecystectomy)has been done on 4.6.1019. But Biopsy (Portion of gall bladder wall for Biopsy) is Adenocarcinoma. For further treatment what are the procedures.

Dear Chhaya, If cancer has spread and cannot be removed, the following types of palliative surgery may relieve symptoms:

  1. Biliary bypass: If the tumor is blocking the bile duct and bile is building up in the gallbladder, a biliary bypass may be done. During this operation, the doctor will cut the gallbladder or bile duct in the area before the blockage and sew it to the small intestine to create a new pathway around the blocked area.
  2. Endoscopic stent placement: If the tumor is blocking the bile duct, surgery may be done to put in a stent (a thin tube) to drain bile that has built up in the area. The doctor may place the stent through a catheter which is a soft hollow tube which is passed into the bladder that drains the bile into a bag on the outside of the body or the stent may go around the blocked area and drain the bile into the small intestine.
  3. Percutaneous transhepatic biliary drainage: A procedure done to drain bile when there is a blockage and endoscopic stent placement is not possible. An x-ray of the liver and bile ducts is done to locate the blockage. Images made by ultrasound are used to guide placement of a stent, which is left in the liver to drain bile into the small intestine or a collection bag outside the body. This procedure may be done to relieve jaundice before surgery.
